Records and Registration
Current students may have their names changed on official university records only after submitting to the Records and Registration office an official document obtained from the Social Security office showing that a name change has been/or is being processed. The form and instructions for applying for a new Social Security card with the name change may be found at http://www.ssa.gov/pubs/10002.html#identity
The Records and Registration office will notify the Student Financial Services, Information Technologies and Payroll offices of any name changes.
Former students may have their names changed by submitting official documentation (marriage certificate, court order, dissolution, etc.) to the Records and Registration office that indicates the name change has occurred.
